Who reviews your screening
Screenings submitted through MyHealthForm.com are reviewed by Scott Strachan, RN BSN, a Registered Nurse licensed in the State of Florida, license no. RN9224129. Florida nursing licenses can be verified through the Florida Department of Health’s license verification portal.
Clinical protocol and physician oversight
Screenings are conducted under a written clinical protocol established and signed by a physician licensed in the State of Florida. The protocol was established on November 22, 2025, and is reviewed and renewed by the overseeing physician every two years.
The protocol defines the screening criteria, the circumstances under which a health statement may be issued, and the circumstances under which a statement must be withheld and the caregiver directed to an appropriate healthcare provider.
Specific screening criteria and decision thresholds are not published, so that the integrity of the screening is preserved.
The signed protocol is maintained on file. Nurse registries, AHCA surveyors, and other appropriate parties may request written confirmation of the protocol and the overseeing physician’s credentials by contacting [email protected].
What the review is
The review is based entirely on the information the caregiver provides in the screening questionnaire. It is a documentation screening — not a physical examination, a diagnostic test, or a medical clearance. No one at MyHealthForm.com examines the caregiver or performs testing.
When a health statement is issued
If the caregiver’s responses meet the screening criteria, a dated, digitally signed health statement is issued stating that, based on the information provided, the caregiver is free from communicable disease.
When a health statement is not issued
If a response identifies a current symptom, a possible exposure, or another concern, a health statement is not issued. The caregiver is directed to obtain evaluation from an appropriate healthcare provider before seeking documentation.
What this covers — and what it does not
The screening is designed to meet the health-statement documentation requirement that applies to caregivers referred by a Florida nurse registry under Fla. Stat. 400.506(6)(a) and Fla. Admin. Code 59A-18.005(6).
It does not replace tuberculosis testing where separately required, routine medical care, or any physical examination required by a licensed home health agency, facility, other employer, or regulation. Nurse registries remain free to accept qualifying documentation from any appropriate healthcare professional.
