How It Works
At MyHealthForm.com, we follow a structured, Florida State-compliant process to demonstrate that in-home caregivers are free from communicable diseases as required by Florida state law. Here's how it works:
Screening Tool Administration
A Registered Nurse (RN) oversees a short, 20-question "Free from Communicable Disease" screening tool completed online by each caregiver. This tool assesses symptoms, exposure, and risk factors related to diseases like tuberculosis, influenza, hepatitis, and COVID-19.
Review and Assessment
The RN reviews your answers to ensure there are no signs or risks of communicable diseases. If any concerns are noted, a form will not be provided and you are referred to your healthcare provider for further evaluation.
Health Statement Issuance
If your answers indicate you are free from communicable diseases, the RN will issue a signed Health Statement confirming your clearance. This statement complies with Florida Statutes 59A-18.005(6) FAC and 400.506(6)(a) FS.
Follow-Up
If you need further evaluation, you'll have up to six months to receive treatment or clearance from a healthcare provider. Once cleared, you can resubmit the screening tool to receive your updated health statement.
Certification
Upon completing the screening with no concerns, you will receive your signed Health Statement, confirming that you meet the "Free from Communicable Disease" criteria and are compliant with state regulations.
