Free From Communicable Disease Health Statement
If you’re an independently employed caregiver working through a Florida nurse registry, you’re required to provide a Free From Communicable Disease Health Statement. MyHealthForm.com lets you complete that requirement online — no clinic visit, no waiting for an appointment.
How it works
Complete a short online screening about your current health and any possible exposure to communicable disease. Your responses are reviewed by a Florida-licensed Registered Nurse under a physician-established protocol. If your screening meets the criteria, you’ll receive a signed digital health statement you can submit to your registry.
If a response raises a concern, we won’t issue the statement — we’ll direct you to the right next step so you can get properly evaluated.
Cost: $55 per screening
Turnaround: Results are immediate for screenings that demonstrate an individual is free of communicable disease. If a response requires further review, you’ll be directed to next steps rather than receiving a statement. Caregivers have up to 6 months to review any concerns with a health professional and can rescreen as many times as needed to demonstrate they are free from communicable disease.
This screening is based on self-reported information and is not a physical exam, diagnostic test, or medical clearance. It’s designed specifically to meet Florida nurse registry documentation requirements under Fla. Admin. Code 59A-18.005(6) and Fla. Stat. 400.506(6)(a).
